Contracted Nurse New

Licensed Registered Nurse (‘Service Subcontractor’) to provide services to children involved in the Illinois child welfare system in the LSSI Central Region of Illinois pursuant to independent contract with LSSI. This is a contracted nursing position for the foster care and intact programs in Central Region- Peoria, Canton, Moline, Galesburg, Champaign, and Danville.

Essential Functions

    

• Provide medical consultation on referred children.
• Assess Medical Needs of children referred, providing an Initial Assessment. Service Subcontractor will monitor progress of Treatment Plan, making recommendations for changes as needed.
• The Service Subcontractor will have a minimum of one face-to-face contact with the child and caregiver at least quarterly.
• Participate in quarterly staffings with LSSI case workers, foster parents, and other involved providers to review progress and needs of children being served, when necessary.
• Track and monitor prescribed medications child, providing updated Medication List to LSSI for the client file.
• Check for appropriate documentation such as timely age-appropriate immunizations, timely regular health, dental, eye and hearing exams, follow-up services for specialty referrals.
• Assist with location of specialists if needed.
• Provide medical training for the foster parent upon request.
• Provide LSSI with nursing notes of all contacts submitted monthly with monthly billings to be filed in the client file required by DCFS to document.
  
  

Service Location:

Services shall be provided at the following locations: office visits, on-site consultation, client's home, etc.

• The Service Subcontractor will ensure there is face-to-face contact with the child in the foster home at least quarterly.
• The Service Subcontractor may see youth in LSSI offices or medical providers' locations.
  
  

Insurance

Service Subcontractor will maintain, or require all the individuals providing services under an negotiated agreement with LSSI to maintain (at Service Subcontractor’s sole expense), valid policies of insurance evidencing general and professional liability coverage of not less than $1,000,000 per occurrence and $3,000,000 in the aggregate, covering the negligent acts or omissions which may give rise to liability for services provided by Service Subcontractor. Service Subcontractor will provide a certificate of insurance evidencing such coverage upon request of LSSI.

Contracted Rates And Reimbursed Expenses

• Initial Assessment: $200
• Quarterly home visit, quarterly assessment, monthly home visit, team meeting: $75/hour
